Monthly Group Rates

Reinforcing your financial cushion doesn’t have to be expensive. MCA&F kept costs affordable by taking advantage of group rates based on the entire membership.

The result is as much as $200,000.00* in AD&D coverage added to your family’s safety net — at a cost that amounts to just a few cents a day.

*At age 70, or if you are already 70, all coverage is reduced by 50%. At age 75, coverage is reduced by another 50%. Spouse’s coverage is 50% of your coverage amount. Monthly rates are shown above. For your convenience, you will be billed on a monthly basis.

Your coverage under the MCA&F AD&D Insurance Plan starts the first day of the month following the date your enrollment form is processed and your first premium payment is received. You cannot be singled out for cancellation, a benefit decrease, or an increase in your premiums. Rates and/or benefits may be changed on a class basis.

Family coverage is a percentage of your coverage; please refer to the family coverage chart below.

Accidental Death and Dismemberment Benefit Amount
If a Covered Person’s Injury results in any of the following Losses within 365 days** after the date of a covered accident the Principal Sum will be paid as outlined below (no more than the Principal Sum will be paid for all losses due to the same accident).

Loss of Life;
Both Hands or Both Feet or Sight of Both Eyes;
One Hand and One Foot; Speech and Hearing in Both Ears;
Either Hand and Sight of One Eye; Either Foot and Sight of One Eye...........100% of Principal Sum

Either Hand or Foot; Sight of One Eye;
Speech; Hearing in Both Ears........................................................................50% of Principal Sum

**In Pennsylvania, 365 days does not apply to Loss of Life. For Oregon residents the Loss of Life must be at least 180 days.

Your amount of the Principal Sum is determined according to what you elected on your Enrollment Form. The amount of Principal Sum for each of your Covered Dependents is determined below as a percent of your Principal Sum.

Family Benefit Coverage

***As determined on the date of accident.

Eligible Member: All members of the MCA&F who are under age 70 and reside in the United States.

Eligible Dependents: Legal Spouse under age 70 and unmarried children who are at least 15 days, but under age 19 (under 25 if a full time student) and residents of the United States.

Additional Benefits:
COMMON CARRIER DEATH BENEFIT:
If an Insured Person suffers a loss of life as a result of an Injury incurred while riding as a passenger on a Common Carrier, the plan will pay 200% of the Principal Sum. Common Carrier means any land, water, or air conveyance operated by a concern, other than the Policyholder, organized and licensed for the transportation of passengers for hire and operated by an employee of that concern. A school bus is considered a Common Carrier.

SEAT BELT BENEFIT: If an Insured Person suffers a loss of life within 90 days of Injury from a covered accident, while: a) a passenger riding in; or b) the licensed operator of, a registered Automobile; and wearing a Seat Belt as verified on the police report, then the Principal Sum payable will be increased by the lesser of $50,000 or 50% of the member's Principal Sum. Accident, for the purposes of this benefit, means the unintentional collision of an Automobile, during which the Insured Person is wearing a Seat Belt.

Automobile means a four-wheeled, private passenger car, station wagon, van or jeep-type vehicle which is not being used as a Common Carrier.

Seat Belt means: a) a belt, lap restraint, or shoulder restraint installed by the manufacturer of the Automobile; and b) a child's car seat properly placed in the Automobile.

EXCLUSIONS: The policy does not cover any loss resulting from: 1. intentionally self-inflicted Injury, suicide or attempted suicide, whether sane or insane (In Missouri while sane); 2. war or act of war, whether declared or undeclared; 3. Injury sustained while riding On any aircraft except a Civil or Public Aircraft, or Military Transport Aircraft; 4. Injury sustained while riding On any aircraft as a pilot or crew member; or 5. Injury sustained while voluntarily taking drugs which federal law prohibits dispensing without a prescription, including sedatives, narcotics, barbiturates, amphetamines, or hallucinogens, unless the drug is taken as prescribed or administered by a licensed physician (For Washington Residents, Exclusion 5. is deleted in its entirety); 6. Injury sustained while committing or attempting to commit a felony (For Maryland Residents, Exclusion 6. is deleted in its entirety); 7. Injury sustained as a result of being legally intoxicated from the use of alcohol. (For residents of Minnesota, Exclusion 7. is deleted and is replaced by the following: 7. Injury sustained while operating a motor vehicle while legally intoxicated from the use of alcohol; For South Dakota residents, exclusion 7. is deleted in its entirety and is replaced by the following: 7. Injury sustained as a result of being legally intoxicated from the use of alcohol and committing an illegal act at the time of loss; For Washington Residents, Exclusion 7. is deleted in its entirety); and 8. a physical or mental illness, or treatment of that sickness.

Injury means bodily injury resulting directly from accident and independently of all other causes which occurs while the Covered Person is covered under the policy. Loss resulting from: a) sickness or disease, except a pus-forming infection which occurs through an accidental wound; or b) medical or surgical treatment of a sickness or disease; is not considered as resulting from injury.

Loss means with regard to: hands and feet, actual severance through or above the wrist or ankle joints; sight, speech or hearing, entire or irrecoverable loss thereof.

Termination: Your coverage terminates on the earlier of: a) the date the policy is terminated; or b) the Premium Due Date on or next following the date you attain the Policy Age Limit shown in the Schedule; or c) the Premium Due Date the required premium contribution is not made, subject to the Grace Period; or d) the Premium Due Date on or next following the date you cease to be a member in good standing with the Policyholder.

In the event of a discrepancy between this information and the policy, the terms of the policy apply. All benefits are subject to the terms and conditions of the policy. Policies underwritten by Hartford Life and Accident Insurance Company detail exclusions, limitations, reduction of benefits and terms under which the policies may be continued in full or discontinued. Complete details are in the Certificate of Insurance issued to each insured individual and the Master Policy issued to the Policyholder. This program may vary and may not be available to residents of all states.
